What to Bring to Your Tax Appointment

 

Personal Information

  • Copy of last year' taxes – This only applies if you’re a new client
  • Correct name(s) first/last (that's on your Social Security Card)
  • Marital Status/Filing Status

 

Dependents

  • Names (that are on Social Security Card), Social Security #'s and Date of Birth
  • Statement of child-care expenses: Total amount paid out, name of recipient, SSN or tax ID #

 

Income

  • W-2's or 1099's from work or any other income sources: Casino winnings, jury duty, etc.
  • 1099’s from banks, credit unions and investment companies regarding interest/dividends earned
  • 1099's from investment companies on stocks SOLD. Please have available the original purchase price and date of purchase.  * For more than 10 stock SALES, please itemize all transactions on a spreadsheet, memory card or some written format.  Include the company name, purchase date, purchase price, sales date, and sales price. *
  • 1099's from any company you’ve taken retirement $'s from; including rollovers

 

Deductions

  • Medical - Totals for un-reimbursed medical expenses: Vision, glasses, contacts, Lasik Eye Surgery, dental, braces, prescriptions, emergency room, ambulance rides, co-pays, etc.
  • Housing - Statement(s) from mortgage companies regarding 2009 interest paid
  • Property tax bill (needed only if amount not on mortgage interest statement)
  • "Settlement Statement" document from refinancing, buying or selling a home in 2008
  • Charity - Weekly or monthly average contributions to church or charity.  Please include average # of bags/boxes of household items, "stuff" donated, any volunteer work, lump sum contributions, etc.  Please include receipts or statements when possible.
  • Loss - Any info regarding a loss due to damaged or stolen property: Car, house, boat, etc.
  • Work Expensespersonal vehicle usage - Average weekly miles (not including commuting) that you use your vehicle for work: Going to different job sites, errands, etc.
  • Parking - Total parking fees paid: Downtown, Airport, Hotels, UNM, Etc.
  • Travel - Expenses related to out-of-town and/or overnight travel that weren't reimbursed
  • Meals & Entertainment - During or for work: Donuts, bagels, luncheons, etc.
  • Misc. - "Other" expenses such as clothes, uniforms, boots, tools, toolbox, equipment, seminars, workshops, education, licensing, home usage, phones, organizers, luggage, union dues etc.

 

Small Business

  • Business income received / 1099's paid to the business
  • Expense TOTALS by category: Supplies, phone, postage, advertising, vehicle usage, etc.

 

Rentals

  • Please keep all rental information separate from personal tax information to avoid confusion.  First year clients, please be sure to include a copy of last year's taxes.
  • Rental income received
  • Expense TOTALS by category: Supplies, repairs, appliances purchased, utilities, etc.
  • Property information (address, mortgage interest paid, taxes, value of rental, etc.)

 

Miscellaneous

  • Total amount of moving expenses and distance moved if over 50 miles AND there was a job transfer or change in employment.
  • Total higher education expenses paid for you or your dependants and # of years attended
  • Statements of student loan interest paid for the year
  • Year end contribution amount/statement(s) to Traditional IRA's, Educational IRA's, etc.
